    信息的主要内容包括：

美国（来回程航班旅客，包括婴儿，均须输入以下信息）：
1. 护照所列姓名（family name, middle name, first name as shown on the passport）
2. 性别（gender）
3. 出生日期，按照年月日顺序（date of birth in the order of year, month and day）
4. 国籍（nationality）
5. 护照号码（passport number）
6. 护照类别（type of passport）
7. 护照签发日，按照年月日顺序（date of issue for passport in the order of year, month and day）
8. 护照过期日，按照年月日顺序（date of expiry for passport in the order of year, month and day）
9. 居住国(country of residence)
10. 护照签发国(country of issue for passport)
11. 在美联系电话(contact phone in US)
12. 在美住址,具体到街道、城市、州(两字代码形式表示)、邮政编码（destination address in US with details of street or road, city, state(two-letter code) and postcode）

加拿大(仅针对去程航班旅客，包括婴儿)
1. 护照所列姓名(family name, middle name, first name as shown on the passport)
2. 性别(gender)
